An exciting opportunity to join a friendly, patient-focused practice Partnership in Somerset, located within easy reach of Taunton, Bristol and the beautiful Somerset coast.

The practice is multi-disciplinary, dynamic and innovative. Our focus is patient-centred care with lots of engagement with local community. We really enjoy our work, develop new services regularly, and are involved in training across different professions. We like to ensure our work is sustainable by supporting each other and looking after the health and wellbeing of practice team members. Portfolio careers are actively encouraged and there are potential future partnership opportunities available too.

We have abandoned 'duty doctor' sessions in favour of mutual support. We have lots of expertise within the practice and you will be encouraged and supported in your own areas of special interest.  We have a strong focus on training and development, and we equip our team with the skills and knowledge to be successful.  We are currently looking at new models of working to future proof the practice and manage the changing demands on General Practice. You would play a key role in these decisions and the future shape of the practice.

Social prescribing underpins our work and this is supported by our in-house Health & Wellbeing Coach as well as our Village Agent. We have established a number of patient groups including an active PPG, Walking Group and Gardening Group at the practice.

This is a golden opportunity for both newly qualified GPs or experienced GPs to come and join a forward-looking, dynamic, multi-disciplinary team.

Practice Nurse

Job summary

The successful candidate will have the ability to work well both as part of a multi-disciplinary team and autonomously. Treatment room skills, such as venepuncture and wound care would be an advantage, along with knowledge of long term conditions management in the community. We are looking for someone who would be willing to undergo additional training (at Diploma level) in areas where skills require development which will be ongoing.

This role is a fantastic opportunity for either an experienced practice nurse or someone in the early stages of their nursing career who wants to develop/extend their treatment room skills before progressing to general practice nurse and long term conditions management

 

Main duties of the job

The post holder is responsible for the delivery of general practice nursing services care to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

 

About us

All practice staff operate over two purpose built practice sites. The Partnership currently comprises of two full-time GP partners, employing 32 staff. The Partners are currently supported by 4 salaried GPs . We have a team of 5 general practice nurses and 1 Health care Assistant responsible to the Nurse Manager and Advanced Nurse Practitioner.

Ongoing education and development is actively encouraged and supported. The practice has recently worked with Health Education England and Plymouth University to introduce CLiP (Collaborative Learning in Practice) into general practice and hosts students on a regular basis. Redgate and Somerset Bridge were finalists in the 2020 student Nursing Times Awards as student placement of the year.

Job responsibilities

 

Summary of Post

Key Responsibilities

Clinical

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long-term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion using opportunities such as new-patient medicals

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions

Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care

Assess and care for patients presenting with uncomplicated wounds

Support and advise women requesting information relating to family planning needs

Support and manage health needs of women presenting for cervical cytology consultations

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad

Promote and deliver evidence-based care for patients presenting with aural conditions

Assist senior practitioners in providing minor-surgery sessions

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
